How Evolysse™ Form Works
Evolysse™ Form is made with hyaluronic acid (HA), which is a naturally occurring substance in the skin that helps maintain hydration and volume. Over time, skin loses this important acid, leading to sagging, fine lines, and wrinkles. Replacing lost HA with Evolysse™ Form can reverse the appearance of volume loss and lift sagging skin in target areas to create a more youthful and refreshed look.
This dermal filler is also created using proprietary Cold-X™ Technology, which preserves the integrity of the HA molecules during manufacturing and increases treatment efficacy. Additionally, the formula uses fewer chemical cross-linking agents to help it integrate well into the skin tissue with less inflammation and swelling.

What to Expect
Your treatment at the Elizabeth Roche, M.D. Medspa starts with a detailed consultation to go over your medical history, evaluate your facial structure, and identify your treatment goals. We will answer any question you have about Evolysse Form or any of the dermal fillers we use.
Before the procedure, baseline “before” photos will be taken. Numbing cream will be applied to ensure that you are comfortable during the treatment. After 20 mins your face will be thoroughly cleansed prior to the injection to prevent the risk of infection.
During the procedure, Evolysse™ Form is carefully injected into the desired areas. Typically, treatment is complete within 10 to 15 minutes, and you are able to go back to your regular daily activities. We may recommend that you avoid applying any make-up to avoid infection as well as avoiding strenuous activity like heavy lifting or working out for a day or so while you recover. More detailed post injection instructions will be provided to you after your treatment.
Results are immediate and last for about a year after treatment.

How is Evolysse™ Form different from Evolysse™ Smooth?
Both Evolysse™ Form and Evolysse™ Smooth are dermal fillers made by Evolus and use the same Cold-X™ Technology during manufacturing. However, each is designed with its own treatment goals in mind. While Evolysse™ Form is a denser, thicker gel filler that provides structural support in treated areas, Evolysse™ Smooth is a thinner gel that is better suited for fine lines and subtle results.
Who can get Evolysse™ Form?
Evolysse™ Form is FDA-approved for treating adult patients over the age of 22 who are in good general health. However, some patients may not be good candidates for the procedure, particularly if they have had a severe reaction to medications before or are allergic to any of the ingredients in the filler, like lidocaine or gram-positive bacteria.
Does Evolysse™ Form have any side effects?
Like with most dermal fillers, side effects from Evolysse™ Form tend to be mild and self-resolving within a few hours or days. After treatment, you may notice some minor swelling, redness, bruising, or bleeding at the injection site. You can use a cold pack as needed to reduce pain and inflammation until it resolves.
Uncommon side effects from injecting any type of dermal filler include, allergic reactions, nodular formation, or vascular occlusion.
